Bilingual Social Worker – Siler City, NC

Under general supervision, provides comprehensive case management services for all clients and their family members. Responsibilities include initial and on-going assessment of bio-psychosocial factors, healthcare factors and educational needs of these clients with a focus on enhancing our client’s capacity for self-support and self-sufficiency.

• Accurately communicate the social worker’s role, functions and services to clients.
• Provides counseling on all options, and makes referrals, as needed as well as follow-up contacts.
• Follows up at least monthly with program clients.
• Establishes with client specific objectives, & achievable goals (both educational & health care related) relevant to mutually define problems/ issues.
• Plans with client for termination of social worker services, and terminates services appropriately.
• Establishes relationships with clients based on empowering them client to learn/develop appropriate skills needed to meet stated goals.
• Fosters client’s independence.
• Provides counseling education, and case management services, with the focus directed towards achieving better pregnancy outcomes for the client.
• May make home visits to client’s homes and/or other agencies in the community.
• Knowledge of and ability to comply with North Carolina state Presumptive Eligibility guidelines and Medicaid for pregnant Women guidelines for documentation and billing for social worker services.
• Demonstrates the ability to serve diverse patient populations served
• May provide supportive/directive counseling including crisis intervention to clients and families in addressing needs.
• Facilitates counseling or educational groups

MINIMUM QUALIFICATIONS
• Education: Master’s Degree in Social Work (MSW) and at least 1 year experience in Social Work required. Experience in community care/home/public health care preferred.

. Bilingual – Must possess the ability to converse in Spanish and English.

Must be professional, flexible, organized, detail oriented, and exemplify outstanding communication skills and ability to interact with all types of individuals.

PATIENT POPULATION SERVED
Pediatric, Adolescent, Adult, Geriatric, Prenatal. Diverse population of patients.

PHYSICAL DEMANDS/ WORKING CONDITIONS
Requires frequent sitting for long periods, operation of standard office machines and computer. May require lifting of up to 25 pounds. Requires eye-hand coordination and Eye- Hand/eye coordination and manual dexterity are required. Requires the use of office equipment, such as computer terminals, telephones or copiers. Requires normal vision range. Work is performed in an office environment. Contact with staff and external clients/vendors. Normal vision range. Ability to work under stressful conditions. Ability to multi-task. Adaptability to change assignments unexpectedly. Must have a personal vehicle for transportation between sites, to patient homes and to other community networks within our service area. Participation in training sessions outside of PHS. Requires moderate computer skills and use of other office equipment.
